define pulmonary edema
excess fluid in interstitial spaces and alveoli
causes of pulmonary edema
HF, acute coronary ischemia (most common), arrhythmias, valve disorders, excess IV fluids, trauma
2 forms of pulmonary edema
cardiogenic and noncardiogenic
cause of cardiogenic PE
increased pressure in the heart
- CHF
causes of noncardiogenic PE
fluid leaks into alveoli due to leaky capillaries
- ARDS, high altitudes, neurogenic, drugs, PE, viral infection, lung injury, toxics, smoke inhalation, near drowning
S/S
restlessness, anxiety, blood-tinged sputum
S/S of acute
SOB, wheezing, anxiety, frothy sputum tinged with blood, chest pain, palpitations, feelings of drowning
S/S of chronic
- SOB with activity, wheezing, weight gain due to fluid buildup, LE swelling, fatigue, dyspea
S/S of high altitude edema
SOB, cough, fever, frothy sputum tinged with blood, palpitations, chest discomfort, headaches
dx pulmonary edema
xray = primary method
also use pulse ox or blood tests
how does it affect breathing
SOB, wheezing, cough, crackles, increased RR
breath sounds
fine crackles (rales) during inspriation
PT for PE
ROM, aerobic endurance, strengthening, functional activites
PT continued
airway clearance via percussion, coughing, pursed lip breathing, suction, segmental breathing, incentive spirometry, education, energy conservation,
