Define chronic bronchitis
productive cough lasting at least 3 months out of the year for 2 consecutive years
patho of chronic bronchitis
inflammation and scarring on bronchial lining that can obstruct flow and increase mucus production
more pathophys
overproduction of mucus via goblet cells. more diffusion, smooth muscles hypertrophy, impaired ciliary function, decreased elimination of mucus obstructs airways
causes of chronic bronchitis
- cigarette smoke
- acute/chronic viral infections
- bacterial infection
- air pollution
- can have acute exacerbations
compications of CB
infections, respiratory acidosis, hypoxia, polycythemia, cor pulmonale
how does CB affect breathing?
- overproduction of mucus
- airways swell
- alveoli damaged, hard to exchange O2 and CO2
- decreased O2 and increased CO2 increases RR
- SOB
- wet, productive cough
clinical presentation of CB
- cough worse in morning and late evening
- barrel chest
- SOB
- decreased chest expansion
- prolonged expiration
- infections
- cyanosis
- blue lips
breath sounds heard with CB
- absence of sounds
- decreased sound as disease progresses
- wheezing or crackles possibly
PT for CB
- meds not effective, advair is only med approvd for COPD
- chest PT, education, coughing, vibratory positive expiratory pressure, percussion, postural drainage, exercise, yoga
