causes of pleural effusion
CHF, pneumonia, malignancy
- also PE, virus, CABG, cirrhosis, TB
definition of pleural effusion
collection of fluid in pleural space where there is normally only a small amount of fluid to prevent friction as the lung expands and deflates. Any condition that interferes with the secretion or drainage of fluid will lead to PE
transudative PE
fluid substance that has passed thru a membrane or has been forced out of the tissue. CHF
exudative PE
fluid w/ a high content of protein and cellular debris that has escaped from BVs and has been depositied into tissues. it is the result of increased permeability or trauma. Infection, trauma, PE, pneumonia, heart abnormalities
Dx of pleural effusion
chest xray, CT scan, ultrasound
prognosis
cardiac fxn declines with decreased venous return to heart, overall prognosis depends on underlying pathology and causes, can cause atelectasis if left untreated
mechanical effect of PE
fluid applies pressure to lung and prevents it from expanding
clinical presentation
SOB, trachea deviated to opposite side of effusion, chest pain, edema in LE, tachycardia, abnormal pulse
breath sounds
diminished
- egophony (ee)
- over PE areas e will sound like aaayyyy (goat bleat)
treatment
treat the underlying cause! breathing, coughing, postural drainage, percussion, vibration
