Liver, Gall Bladder, Pancreas And Spleen Flashcards

1
Q

What is the ligamentum venosum a remnant of? And where would you find it?

A

The ductus venosus - in the foetus this is used to shunt blood from the umbilical vein directly to the inferior vena cava

Found between left lobe and caudate lobe

Q

What is the falciform ligament and where would you find it?

A

A double layer of peritoneum that separates the R and L lobes of the liver. It attaches the anterior surface of the liver to the anterior abdominal wall

Q

What is the coronary ligament and where would you find it?

A

Double layer of peritoneum that attaches the superior surface of the liver to the diaphragm.
Anterior coronary ligament runs along surface of liver off of diaphragm. Posterior coronary ligament runs from both ends of anterior CL surrounding the porta hepatis

Q

Where do you find the triangular ligaments?

A

Found where anterior and posterior coronary ligaments meet. L triangular found on outermost point of superior surface. R triangular ligament found on the outermost point of the posterior surface of the bare area.

Q

What is the role of the liver?

A

Filter and detoxify blood from chemicals and drugs. It also makes proteins important for blood clotting.

Q

What is the role of the gallbladder?

A

Concentrates and acts as a reservoir for bile. Bile is released through the cystic and then common bile ducts into the duodenum as a response to cholecystokinin secretion

Q

Describe the flow of bile

A

Bile is made in the liver and stored in the gallbladder. Bile comes out of the liver from the L and R hepatic ducts which come together to form the common hepatic duct.

Common hepatic duct joins with cystic duct to form the common bile duct.

Common bile duct along with pancreatic duct form the hepatoduodenal ampulla of Vater which then enters second part of duodenum

13
Q

How are gallstones formed?

A

Most commonly composed of a mix of cholesterol and bile pigment, formed due to an altered composition of bile resulting in cholesterol crystals. Often asymptomatic but when they block the exit from the gallbladder they cause complications.

16
Q

What is anterior to the pancreas?

A

The lesser sac and the stomach

17
Q

What is posterior to the pancreas?

A

IVC, aorta, coeliac plexus, left kidney and left adrenal gland

18
Q

What is the blood supply to the pancreas?

A

The head is supplied but the superior and inferior pancreaticoduodenal arteries. Body supplied by a branch of the splenic artery

19
Q

What is the blood supply of the liver?

A

Proper hepatic artery (derived from common hepatic artery) and hepatic portal vein (main supply which carries deoxygenated blood allowing the liver to carry out its detoxification)

20
Q

What is the function of the spleen?

A

Filters blood, removes old red blood cells and has a role in immunity

21
Q

Describe the location of the spleen

A

L hypochondrium.
Stomach is anterior to it.
Diaphragm, left lung and ribs 9-11 posterior to it.
Splenic flexure inferior to it. Left kidney and tail of pancreas medial to it.

22
Q

What is the blood supply to the spleen?

A

Splenic artery

23
Q

Describe the location of the gallbladder

A

Right hypochondrium inferior to the liver

24
Q

What is a porto-systemic anastomoses and why does it occur?

A

An alternative route for blood to take when the direct blood through the liver becomes congested. Causes include cirrhosis due to alcoholism
