Consciousness
Level of awareness of oneself and the environment. Appropriate response to external stimuli; oriented to time, place, person, and purpose
Confusion
Inappropriate response to stimuli and decreased attention span and memory; inappropriate reactions to simple commands
Lethargy (hypersomnia)
Drowsiness or increased sleep time; is able to be aroused; responds appropriately; possibly falls asleep again immediately
Delirium
Confusion, disordered perception, and decreased attention span; motor and sensory excitement; inappropriate reactions to stimuli; marked anxiety
Coma
Loss or lowering of consciousness
Coma Stage I (stupor)
Arousable with vigorous repeated stimuli; slowed verbal response but deep tendon and superficial reflexes are intact
