Objective data
perceived by the nurse, what the nurse sees, hears, measures, and feels
Examples: vital signs, abnormal lung or heart sounds
Subjective data
nurse encourages a full description by the patients, usually put into “quotation marks”
Assessment
an evaluation of appraisal of the pts condition
Nursing Assessment
consists of subjective and objective data collection
Focused Assessment
attention is directed to a particular part of the body where signs and symptoms are localized or most active
Turgor
elasticity of the skin caused by the outward pressure of the cells and interstitial fluid
Pitting Edema scale
1+ : barely perceptible
2+ : rebounds in 10-15 seconds
3+ : lasts for 30 seconds - > 1 minute
4+ : lasts as long as 2 - 5 minutes
