Assessment
a systematic, dynamic way to collect and analyze data about a client, the first step in delivering nursing care
Nursing Process
a systematic method by which nurses plan and provide care for patients
Assess, Analyze (Diagnose), Plan, Implement, Evaluate
Diagnose
to identify the type and cause of a health condition
Medical Diagnoses
the identification of a disease or condition with the evaluation of physical signs, symptoms, patient interviews, laboratory tests, diagnostic procedures, review of medical records, and patient history
Planning
Phase of the nursing process, priorities of care are established and nursing interventions are chosen to best address the patient problem statement
Patient Problem Statement
address human responses to health problems and life processes
the patient problem statement may change or resolve as care is provided
Patient goal statement
indicates the degree of wellness desired, expected, or possible for the pt to achieve
Nursing Interventions
activities that promote the achievement of the desired patient goal
Example of Pt Problem and Goal Statement
Pt Problem Statement: Potential for compromised skin integrity due to the pressure on bony prominences, impaired circulation and physical immobilization
Pt Goal Statement: Pt will remain free of any skin impairments, such as reddened areas by 10/28/2018
Implementation
Phase of the nursing process the nurse and other members of the team put the established plan into action
Evaluation
determinations made about the extent to which the established goals have been achieved
