Lecture 4B - 1st Trimester (Late) Flashcards
In the first trimester late scan, what additional patient history question do you ask (in addition to questions asked to at dating scan)
previous pregnancies with chromosomal abnormality
what fetal anatomy features can you visualize by 11-14 week (late 1st trimester scan) (4)
skull
nasal bone
jaw angle
chin/chest separation
what late 1st trimester measure correlates to GA?
biparietal diameter (BPD)
how would you measure BPD
outer to inner
what is the hindbrain called and what does it look like
rhombencephalon - posterior anechoic structure
what are the main measurements in 1st trimester late
biparietal diameter
nuchal translucency
femur length
when is nuchal translucency assessed?
assessed between 11 w - 13w 6 days (14 weeks too late)
nuchal translucency measurements are valid ONLY IF ……
CRL is between 45-84 mm
NT should measure
<3 mm
what is a pitfall to measuring NT?
potential to include amnion, should show as separate
how do you measure NT? (landmarks and actual measurement location)
perfectly midline with nasal bone, head and chest in image
INNER TO INNER at widest
what should NOT be in NT image
no zygomatic bone, no eye, no flex/extension of neck
what does increased NT indicate
indicates increased risk for chromosomal abnormalities (trisomy 13,18,21)
NT measurement increases with (2)
GA and maternal age
what does maternal serum screening measure? (2)
b-hCG and PAPP-A
