Explain the characteristics of granulomatous and granuloma and differentiate between them:
These are characterized primarily by fused macrophages that have formed multi-nucleated giant cells (Langerhans giant cells). Granulomatous = diffuse
Granuloma = nodular
Describe the pathology of a granuloma going from the inside to the outside:
Center: central caseous necrosis due to hypoxia and free radical injury, central clusters of epithelioid macrophages, fusion of epithelioid macrophages to form Langerhans giant cells
Middle layer: T lymphocytes (could include plasma cells)
Rimmed by: fibroblasts (older lesions) and also connective tissue
What is it important to eliminate tuberculosis as a differential diagnosis?
It has significant public health significance
