What is induction treatment?
treatment to induce complete remission
What is consolidation treatment?
treats sub-clinical residual disease with intent of increasing cure rate or survival
What is maintenance therapy for chemo?
long term low dose therapy to decrease recurrence/progression rate
What is adjuvant chemo therapy?
chemo after primary surgery or radiotherapy to increase survival/cure rate
What is neoadjuvant chemo therapy?
chemo before primary surgery or radiotherapy to increase cure rate or survival or decreasing morbidity with primary therapy
What is palliative chemo therapy?
chemo to alleviate symptoms with intent of prolongation of life or cure
What is the purpose of combination chemo?
allow maximum tumor kill with least side effects
What is the mech of action of alkylating agents?
cross link DNA, RNA, or proteins
Do alkylating agents require cell proliferation?
yes, require cell proliferation even though not cell cycle specific
What is mech of action of resistance to alkylating agents?
resistance via enhanced DNA repair of glutathione conjugation

cyclophosphamide

What are 2 side effects of cyclophosphamide?
myelosuppression

severe myelosuppression
pulmonary fibrosis
hyperpigmentation
What is the mech of action of cisplatin?

What are 2 side effects of cisplatin?
- nephrotoxicity
- acoustic nerve damage
What can you give with cisplatin to prevent nephrotoxicity?
amifostine and chloride diuresis
What is mech of action of ionizing radiation?
induces double stranded DNA breaks
What are antimetabolite neoplastic drugs?
analogs of natural metabolites in RNA and DNA synthesis
compete with normal metabolites for enzymes or get incorporated into DNA/RNA
S phase specific
What is mech of action methotrexate?
folic acid analog that inhibits dihydrofolate reductase –> less dTMP –> less DNA and protein synthesis

Why do you give leucovorin with methotrexate?
leucovorin = folinic acid
rescues the BM from myelosuppression caused by methotrexate
What is mech of action 5-fluorouracil [5-FU]?
pyrimidine analog bioactivated to 5F-dUMP
complexes with folic acid and inhibits thymidylate synthase –> less dTMP –> less DNA and protein synthesis
