What is laryngospasm?

A

Form of airway obstruction caused by involuntary contraction of the vocal cords

What are the clinical features of laryngospasm?

A
  1. Inspiratory stridor
  2. Desaturation
  3. Bradycardia
  4. Cyanosis
  5. Increased inspiratory effort/tracheal tug
  6. Paradoxical chest/abdominal movement

What are possible etiologies of laryngospasm?

A

Airway irritation/obstruction 2/2 blood or secretions
aspriation or regurgitation
excessive stimulation (e.g. suctioning)
light anesthesia

What are possible etiologies of stridor?

A
  1. Laryngospasm
  2. anatomic/acquired malformations (foreign body, large tonsils, large epiglottis, congenital)
  3. infection
  4. neoplasms
  5. neurologic disorders - bulbar palsy, MG
  6. trauma
  7. allergic reaction
  8. vascular rings

What is the management of laryngospasm?

A
  1. Stop offending stimulation/surgery; consider need for help
  2. 100% oxygen
  3. Open airway (jaw thrust)
  4. Deepen anesthesia
  5. Positive pressure mask ventiltion
  6. If unsuccessful, treat with sux
  7. If bradycardia, use atropine
  8. If no improvement, intubate/ventilate
