Define hypercalcemia.
A
Hypercalcemia is defined as elevated calcium levels in the blood above the normal range of 1 to 1.25 mmol/L

What are the clinical features of hypercalcemia?
A
- asymptomatic
- nausea, anorexia
- emotional lability, lethargy, stupor, coma
- polyuria, kidney stones, hypovolemia
- hypertension, heart block, arrhythmias
- osteoporosis, bone pain

What is the differential diagnosis for hypercalcemia?
A
- hyperparathyroidism - primary, secondary, tertiary
- vitamin D intoxication
- malignancy
- granulomatous disease (sarcoid, TB)
- medications (thiazide diuretics, lithium, milk-alkali syndrome)
- endocrine (hyperthyroidism, adrenal insufficiency, acromegaly, pheochromocytoma)
- Immobilization
- decreased calcium excretion (i.e., hypocalciuric hypercalcemia)

What is the management of hypercalcemia?
A
- assess and treat underlying cause
- IV hydration with NS
- Once volume replete, induce diuresis with loop diuretic
- hemodialysis for life-threatening hypercalcemia
- consult endocrinology regarding other treatments (bisphosphonates, glucocorticoids, calcitonin)
