Patients eyes closed, palm open, doctor draws #, shape, or letter in patients palm, tell patient which category it is and see if they can I.D. the symbol
Sensory Exam: Sharp vs. Dull
Patients eye’s closed, Dr. uses NEUROTIP to touch 5 points on each extremity alternating sharp/dull, patient identifies sensation.
Motor Exam: Passive Elbow Motion
Dr. moves elbow through: Flexion, Extension, Supination, Pronation
Motor Exam: Trapezius Muscle Test
“Shrug and hold while I push down, resist.”
Coordination & Gait: Patting Test

Palpate for patella and strike w/ flat edge of hammer.

C.N. Exam: A patient presents in your office with sharp stabbing pain on the right side of his face. He states that when he chews he feels weakness in the right side of his mouth.
- STATE APPROPRIATE C.N. TO BE TESTED.
- PERFORM ALL PROCEDURES & STATE ALL SENSORY, MOTOR AND INTEGRATION CENTERS FOR ANY REFLEX PERFORMED.
V: Trigeminal (Ophthalmic, Maxillary, & Mandibular)
- Clench teeth
- Pain discrimination
- Light touch to face
- Corneal reflex
- Light touch to ant 2/3 of tongue
- Oculocardiac Reflex
Nerve Root Eval: A patient complains of numbness on the back of the left calf and side of the left foot. He also states that when he tries to use the clutch in his car, his left foot doesn’t seem to be able to press very hard.
- STATE APPROPRIATE NERVE LEVEL
- STATE APPROPRIATE DISC LEVEL
- PERFORM RELATED MUSCLE TESTS (stating muscles & peripheral nerve innervations), DEEP TENDON REFLEX & SENSORY EXAM
Nerve Root: S1
Disc Lvl: L5
- Plantar Flexion = Gastrocnemius and Soleus (tibial n)
- Plantar flexion & Eversion = Peroneus longs and brevis (superficial peroneal n.)
- Hip Extension: Gluteus maximus (inferior gluteal n.)
- Reflex: Achilles
- Dermatome: L5,S1,S2 (Posterior aspect of leg, lateral foot, lateral aspect of little toe)
Ortho Exam: A patient complains of radiating pain down the posterior thigh and leg.
-STATE ; PERFORM AN APPROPRIATE ORTHO TEST TO EVALUATE THE PATIENT
Straight Leg Raiser
- Radiating pain &/or dull posterior thigh pain
- Pain occurring between 30 and 70 degrees = discogenic sciatic radiculopathy
- Pain beyond 70 deg = tight hamstrings
