L4 Outcome Measures Flashcards
Why do we use outcome measures?
Track patient progress
Justify visits to insurance
Screen for problems
Discriminate among impairments
Predict future events
Show value of PT
Research
How to select correct outcome measure
Meets the needs of patient
valid
reliable
responseive to MDC and MCID
Types of measures
Patient reported outcome emasures–> shows patient perspective, but relies on their ability to recall
Performance: high reliability
Outcome measure frequency
Acute: daily or 2-3 days
IPR: once a week
OP: once a month
Home health: once a month
Outcome measures for neuro individuals
Berg
FUnctional gait
Balance confidence
10 meter walk
6 min walk
5 times sit to stand
What is EDGE?
Evidence database to guide effectiveness
systematically review measurement qualities of commonly used tools
What ICF category do the Core measures fit into?
Activity
Choose from the ____ set first and supplement with other relevant ____ after
CORE OUTCOME
EDGE
edge includes dynamic balance, gait speed, gait endurance, patient reported outcome measure, sit to stand transfer
Select measures on needs vs goals?
Select outcome measures based on their GOALS and capacity to improve
Standardization importance?
standardization helps to administer and score in the same way. Ensures reliability of the measure
Should you measure a patient that won’t do well on a test?
YES
it is important to assess baseline in acute and post acute even if they score a 0
low scores are useful in those with capacity for change
Patient doesn’t want to do outcome measure…
Educate that measurement helps us to make decisions, see the progress, allows patient to tell family how they are doing
create things like set benchmarks or days for patient to ge ttested
as the PT, be selective and choose the ones that are the most importatn
Writing Goals for neuro injuries
write for the current episode of care. Create goals that are attainable for NOW
Acute–short 1-2 days, Long 7-10 days
Sub acute–S 1 week, L 3-4 wks
OP: S 1 month, L 6-12 mo
Goals should
- address identified activity and participation limitations
- Address goals of patient
- Consider capacity for recovery
- Be SMART
Patient Specific Functional Scale
uses performance of an activity to measure success. Amount of assistance, distance, quality, etc are examples
It allows pt to select the activity they want to imporve
broadesn the types of activities that can be selected for POC
