Physiatrists
maximize what a pt can do and assist the pt in adapting to what he or she cannot.
use a variety of techniques including meds, injections, modalities, prosthetics, therapy, etc
Rehabilitation nurse
restore pateints lives so they have freedom and independence again.
includes administrator, clinical nurse leader, specialist, consultant, NP
Health Care social worker
provides families and groups with psychosocial support
includes advising family caregivers, case and care management, interventions designed to promote health, prevent disease, and address barriers
Neuropsychologist
specialty field within clinical psychology, dedicated to understanding the relationships between brain and behavior
DME Provider
help produce and provide mobility devices, ADL devices, environmental devices, positioning devices
Assistive Tech Professional, Seating Mobility Specialist
work with team to find best match of mobility and other AT devices to promote independence for patients
Orthotist/Prosthetist
work with team to provide orthotics and prosthetics to optimize patient independence with mobility and positioning
Goal of IPR Discharge Planning Meeting
determine a SAFE discharge plan for a patient
need to establish D/C destination, timeline, goals, DME needs, environmental adaptations, CGT
