L12: Diverticular Disesease And IBD Flashcards
What is diverticular disease
Outpouching of the bowel wall
What layers of the bowel are involved in Diverticular disease
The full component of the bowel
What is the main area of the colon that can become affected
Sigmoid colon
Descending colon
Any part of the bowel
What are the causes of diverituclar disease
Unknown
What are the factors linked to the cause of Diverticular disease
Old age
Developed countries
Low fibre diet
Constipation and small faecel volume
How does diverticular disease occur
It occurs in weakness of the bowel wall i,e where the vessels enter, when you create a high pressure across the colonic wall it will pop the bowel wall outwards and form a pouch
How can we investigate diverticulitis
Ct scan
Colonoscopy
Ultrasound
Cystoscopy- too see faecal material in the bladder
What can diverticular disease cause
Rectal bleeding
Diverticulitis e.g infection
Pericolic abscess
Fistula to the bladder or stricture
What is the management of Diverticular disease
Avoid straining
High fibre diet
Diverticulitis: antibiotics, analgesia and fluid
Abscess, ritual and perforation- surgery
How does diverticulitis present
Left side pain, change in bowel habit, fever
What is irritable bowel syndrome
A disorder of the GI function in the absence of known pathology or structure
What criteria is used for ibs
Rome 111 criteria
What does the Rome 111 criteria involve
A recurrent abdominal pain or discomfort for at least 3days per month in the last 3 months associated with 2 or more of:
- Relieved by defecation
- Change in frequency of stool
- Change in consistency of stool
What are the supportive symptoms in ibs
Less than 3 bowel movements per week More than 3 bowel movement per week Abnormal stool form Lumpy hard stool Watery stool Straining Urgency and bloating
What are the subtypes of ibs
Ibs-d diarrhoea
Ibs c- constipation
Ibs m-mixed
What is the cause of ibs
Unknown
What is coeliac disease
Lactose intolerance
What are the symptoms for coeliac disease
Diarrhoea
Bloating
Abdominal discomfort
What is the management of ibs
Give diagnosis and reassurance
Listen to concerns
Lifestyle advice
Placebo
What type of diet would be useful for constipation
High fibre diet
What type of diet would be helpful dope diarrhoea
Lactose exclusion
What are the other pharmacological management for ibs
Antiaspasmodics: to reduce the pain by relaxing smooth muscle
Tricyclics antidepressants for pain relief
Loperamide or codiene to treat urgency and diarrhoea
Isphagula husk for constipation who cannot tolerate increased fibre
Linaclotide- if laxative not effective
What is malabsorption
Defective mucosal absorption of nutrients
What is coeliac disease
Sensitivity to gluten
What is the presentation of coeliac disease
Failure to thrive Anaemia Mouth ulcers Haematinic deficiency Antibody test- TTG, endomysial/gliadin are positive and serum iga Duodenal biopsy
What is the gold standard for coeliac disease
Duodenal biopsy
What will coeliac disease show in the duodenal biopsy
Villous atrophy
What is the treatment of coeliac disease
Gluten free diet
Replace haematinic and calcium
What is pancreatic insuffiency
Due to alcohol, pancreatitis, cystic fibrosis and pancreatic tumours
What is the presentation of pancreatic insuffiency
Steatorrhea- cant absorb fat or protein therefore stools are fatty
Weight loss
What is the diagnosis of pancreatic insufficiency by
Ct abdomen scan- shows atrophy and calcification of pancreatic parenchyma
Faecal elastase test
What is the treatment of pancreatic insufficiency
Pancreatic enzyme supplements
What is giardia lamblia the main cause of
Travellers diarhoea
What investigations can be carried out for giardia lamblia
Low b12
Iron deficiency
Stool culture
Duodenal biopsy
What is the treatment for giardia lamblia
Metronidazole
What is Whipples disease presentation
A rare disease characterised by: Pyrexia Arthralgia Weight loss Malabsorption
What will the blood test investigation show in Whipples disease
Anaemia of chronic disease
B12 and folate are normal
What is the diagnosis of Whipples disease by
By duodenal biopsy that will show PAS positive macrophages
