what is a medial branch neurotomy?
- radiofrequency abalation of medial branch of dorsal rami
- under fluoroscopy
- indicated for pain relief to address z-joint pain
what is a Laminoforaminotomy?
- Tissues in neuroforamen compressing nerve tissue removed
- lamina
- disc
- hypertrophied ligaments
- under fluoroscopy
- open vs minimally invasive
what is a laminectomy?
- removal of lamina
- complete → removal of lamina and SP
- likely contributes to diminished stability
- tripod effect → more force directed to Z-joints
what is a laminoplasty?
- reconstruction of posterior ring at lamina
- open door → bone graft from SP fixated on open side
- indications:
- multi-level spondylosis/spinal stenosis
- increases space for cord
- posterior approach, removal/thinning of lateral lamina
what is percutaneous discectomy?
aspiration of nucleus via probe
indications → HNP/disc origin of symptoms
what is a microdiscectomy?
removal of disc that is compressing/irritating the nerve root
up to 90% success rate reported
what is an interbody fusion?
- indicated for stenosis
- reduces/eliminates segmental motion/stress on invovled structures
- two types:
- TLIF
- ALIF
- increased risk for subsquent degeneration of adjacent segments
what is a TLIF?
transformainal lumbar interbody fusion
bone graft and titanium mesh placed into IV space
what is a ALIF?
anterior lumbar interbody fusion
fixation with bone grafts, cages, dowels
what is an arthroplasty?
disc is removed and replaced with metal and plastic prosthesis
restoration of motion
avoidance of subsequent adjacent segment stress concentrations
