Knee Assessment and Pathology Flashcards
What is patella tracking?
The force of the quads and the force on the tibia tendon causes the patella to want to shift laterally
When happens at the knee when kicking a ball?
Anterior roll and anterior slide
What happens at the knee when standing up?
Anterior roll and posterior slide
What happens to the tibia during knee extension?
Anterior translation, ACL taut
What happens to the tibia during knee flexion?
Posterior translation, PCL taut
What is the screw home mechanism?
Occurs at the end of knee extension, and between full extension and 20 degrees of knee flexion
What muscles are involved in medial rotation of the knee?
Semitendinosus, semimembranosus, gracilis, sartorius, popliteus
What muscle is involved in lateral rotation of the knee?
Biceps femoris
What is the mechanism of an ACL injury?
- Anterior tibial translation, knee hyperextension, knee external rotation and valgus
- Landing with valgus knee
- Change in direction with fixed foot and external rotation
What is the mechanism of a PCL injury?
Impact of anterior tibia with flexed knee
What is the mechanism of an MCL injury?
Direct valgus force via contact, or with an ACL injury
Does a grade II MCL tear require surgery?
Not necessarily, mainly grade III
What is the mechanism of an LCL injury?
Varus force from direct contact or severe hyperextension
What is the mechanism of a meniscus tear?
Twisting with foot fixed, greater with knee flexion
What are common signs of a patella tendinopathy?
Dull ache following high impact, pain during eccentric loading
What typically occurs in females with patellofemoral pain syndrome?
Weak hip muscles, weak thigh muscles, tracking of knee cap when bending
What is Osgood-Schlatter disease?
Non-articular osteochondritis, softening of avulsion of growth plate resulting in a hardening on the tibial tuberosity
Occurs during childhood
What is Iliotibial band syndrome?
Compression of fat pad that causes the pain. Occurs as the knee flexes to 30 degrees and the ITB moves posterior to the lateral femoral condyle. When it extends it moves posterior
What are typical signs of knee osteoarthritis?
Pain with compressive loading, restricted ROM, reduced knee extension strength
