IV fluid prescribing - renal Flashcards
5 Rs of fluid prescribing
Resuscitation Replacement Routine maintenance Redistribution Reassessment
resuscitation
haemodynamically unstable
replace deficit and cover ongoing losses
large boluses of fluid repeatedly
routine maintenance
no deficits
work out how much they would normally need
replacement
have abnormal losses but are not yet shocked
NICE guideline for IV fluid therapy
useful resource
what do you give in fluid resuscitation
500ml bolus fluid stat
routine maintenance volumes for fluids, Na, K and glucose
25-30ml/kg/day
1mmol/kg/day Na and K
50-100g/day glucose
crystalloid/colloid fluids are better for resus
crystalloid
examples of crystalloid fluids
saline 0.9%
Hartmanns (balanced crystalloid)
which fluid can cause hyperchloraemic metabolic acidosis
NaCl 0.9%
what fluids would you give someone with hyperkalaemia
sodium chloride 0.9%
because Hartmanns has K in it
dextrose is essentially water
yes
therefore, never used for replacement or resuscitation
example of colloid fluid and when would you give this
albumin 4-5%
only in patients with SEVERE sepsis for fluid resuscitation
4 groups of commonly used blood products
Packed red cells - PRCs
Platelets
FFP
Cryoprecipitate
