Midazolam
BDZ; increase GABA channel opening
premed: sedate, anxiolysis, amnesia
preintubation
minimal CV effects, decrease resp. response to hypercapnea
General opioids
Morphine, Fentanyl, Remifentanil, Naloxone
minimial CV effects, decrease resp. drive and increase hypercapnea tolerance
Thiopental
Barbituate; prolong GABAr open
decrease BP, increase HR, maintain CO
neuroprotective: decrease ICP, CBF, CMR
long elimination: 8-12 hrs
Etomidate
increase GABAr affinity
slight CV increase, minimal resp. effect
Propofol
slows channel closing
eliminated in 30-60 min (quick)
CV, Cerebral, and Resp. effects same as others
Ketamine
NMDA receptor antagonist
dissociate thalamus from limbic system = dissociative anesthesia
CV: increase BP, HR, CO
mimimal resp. and cause flashbacks, increase ICP
Dexmedetomidine
A2 adrenergic agonist, inhibit Adenylate cyclase and NE release
biphasic CV, preserve hypercapnea response
sedative, decrease opioid and anesthetic requirements
