Ischemic Heart Disease- Angina Flashcards
Types of angina?
Stable angina,
Prinzmetal (variant) angina
Unstable (crescendo) angina
stable angina caused by?
atherosclerosis
Prinzmetal (variant) angina caused by?
coronary artery vasospasm
Unstable (crescendo) angina caused by?
disruption of atherosclerotic plaque with partial
thrombosis in coronary artery
ANGINA Pathophysiology?
All types of angina caused by transient ischemia resulting in inadequate myocardial oxygenation
stable angina clinical signs?
- Precordial chest discomfort associated with exertion;
- pain is relieved by rest and
nitroglycerin; - nonspecific ST-T changes on ECG
Prinzmetal angina clinical signs?
- Episodic chest pain occurring at rest;
2. may see ST elevation on ECG
Unstable angina clinical signs?
- Progressively frequent chest pain initially occurring with activity but later occurring at
rest; - pain takes increasingly longer time to be relieved
- considered indication for acute MI in near future;
- may see ST depressions on ECG
stable angina treatment?
Nitrates;
β-blockers;
statins;
aspirin
Prinzmetal angina treatment?
Vasodilators (nitrates, calcium channel blockers)
Unstable angina treatment?
β-blockers; statins; aspirin; coronary evaluation with catheterization in near future; consider heparin treatment
Angina pectoris may be clinically silent in?
Diabetes
