Acute Myocardial Infarction Flashcards
1
Q
Acute Myocardial Infarction causes?
A
vasospasm,
embolus,
atherosclerotic thrombus resulting in coronary artery
occlusion
2
Q
Acute Myocardial Infarction Risk factors?
A
increasing age, hypertension, smoking, diabetes, male gender, postmenopausal women, hyperlipidemia
3
Q
POST AMI micro fx?
A
- below 12 hrs= no micro change visible
- 12-24 hrs= infarct area brightly eosinophilc, myocytes anucleate, interstitial edema
- 1-3 Days= Neutrophilic Infiltration
- 3-10 Days= macrophagic infiltrate, granulation tissue at periphery of infarct, progress towards centre
- 10 days- 2 months= scar formation
- after 2 months= fibrous scar
4
Q
POST AMI macro fx?
A
- below 12 hrs= no macro change visible
- 12-24 hrs= pale with blotchy discolouration
- 1-3 Days= soft, pale, yellow area
- 3-10 Days= hyperemic border around yellow area
- 10 days- 2 months= scar formation
- after 2 months= fibrous scar
5
Q
Acute MI clinical signs?
A
- Crushing, substernal chest pain with radiation to jaw and left arm;
- dyspnea;
- nausea;
- diaphoresis
6
Q
Acute MI complications?
A
- cardiac arrhythmia (can cause sudden death within first few days),
- fibrinous pericarditis (within 3-5 days),
- pulmonary edema,
- CHF,
- shock,
- thromboembolism,
- rupture of
ventricular free wall or septum (VSD) (within 7-10 days), - rupture of papillary muscle leading to
mitral regurgitation,
Dressler syndrome (autoimmune fibrinous pericarditis, 1-3 weeks post-MI)
7
Q
Acute MI Treatment?
A
Thrombolytic therapy and/or coronary angioplasty for early MI
