What are contraindications for induction?
- Malpresentation
- Placenta praevia/vasa praevia
- Prolapsed umbilical cord
- Foetal distress
- Anatomical abrnomalities (e.g. pelvic tumour)
What medications are used for induction?
- Topical prostaglandin analogues
- IV synthetic oxytocin
What are the possible adverse effects of drugs used to induce labour?
- Hypotension
- Hyponatraemia
What Bishop’s score would indicate towards induction?
Less than 5
How is a fetus monitored in low risk labours?
Intermittent auscultation
How is a fetus monitored in higher risk labours?
Cardiotocography (CTG)
What are some indications for CTG monitoring?
- Induction of labour
- Post-maturity (>42w) or prematurity (<37w)
- Multiple pregnancy
- Underlying maternal health conditions
- Haemorrhage
- Abnormal auscultation
What are the main indications for a cesarian section?
- Foetal distress
- Failure to progress/failed induction labour
- Malpresentation
- Severe pre-eclampsia
- Placenta praevia
What is a Category I section?
Emergency (within 30 mins), there is an immediate threat to life of woman or foetus
What is a Category II section?
Urgent (within ~90mins), maternal or foetal compromise but not life threatening
What is a Category III section?
Scheduled (No time limit) requiring early delivery but no compromise
What is a Category IV section?
Elective (no time limit) at a time to suit woman and maternity team
