Active Listening
the nurse is actively engaged in receiving and decoding messages that patients send. The nurse listens to the patient with two purposes in mind; to comprehend the message and evaluate its meaning
Empathy
intellectually identifying with the way another person feels
PQRSTU Mnemonic
Provoking factors: What brought on the symptoms? What were you doing when this started?
Quality: Is the pain stabbing, burning, sharp? Is it a gnawing feeling?
Radiation: Does the pain go anywhere else or does it just stay in one place?
Severity/ symptoms: how bad is it? Are there any other symptoms associated with it?
Timing/triggers: When did it come on? Is it continuous or intermittent?
Understanding Patient’s Perception:
Health Assessment
A complete health assessment also includes gathering information about a person’s medical history and lifestyle, doing laboratory tests, and screening for disease.
Physical Exam
A physical examination is an evaluation of the body and its functions using inspection, palpation (feeling with the hands), percussion (tapping with the fingers), and auscultation (listening).
