What are the characteristics of IPF .
A
- Usually elderly male >65 years
- Presents with progressive dyspnoea,dry cough, diffuse bilateral crackles (creps) often high pitched called Velcro, >2/3 have digit clubbing.
Remember opportunistic lung infections in HIV can also present us an interstitial disease lung

Explain the too types of respiratory failure mechanisms
A
Type 1= reduced PO2 with normal PCO2
This mechanisms is a v/q mismatch which responds to oxygen
Type 2 = reduced PO2 and increased PCO2= shunt does not respond well to oxygen needs ventilation (PEEP/CPAP)
