Most common causative organism?
Epstein-Barr Virus
Triad of symptoms?
Sore throat, fever and lymphadenopathy
Where can lymphadenopathy occur?
Anterior and posterior triangles of the neck
What can be seen inside the mouth?
Palatal petechiae
Other less common causative organisms?
Cytomegalovirus and HHV-6
Other symptoms?
- malaise
- anorexia
- headache
- splenomegaly (occurs in 50% of patients)
- hepatitis, transient rise in ALT
- lymphocytosis
- haemolytic anaemia secondary to cold agglutins (IgM)
What may happen if patient is given an antibiotic e.g. ampicillin or amoxicillin?
99% patients develop a maculopapular, pruritic rash
How long does it take for symptoms to typically resolve?
2-4 weeks
Diagnostic test?
Heterophil antibody test (monospot test) and FBC in the second week of illness
Management?
Rest, analgesia
Advice on contact sports?
Avoid playing contact sports for 8 weeks due to increased risk of splenic rupture
