Which organism causes Gonor?
Neisseria gonorrhoeae
- gram negative diplococci
Presentation of gonor in women?
**Green discharge
dysuria
abnormal bleeding **
skene’s gland / bartholin’s gland affected
systemic / extragenital:
- pharyngitis
- rectal pain + discharge
- disseminated infection
How to invesitgate gonor?
Vulvovaginal swab analysed by NAAT
microscopy : gram - diplococci within polymorphnuclear leukocytes
What will disseminated gonococcal infection look like
Bacteraemia - septic arthritis, rash, tenosynovitis, skin lesions, meningitis
Liver complication gonorrhoea
fitz hugh curtis = perihepatitis pain
How to manage gonorrhoea?
Ceftriaxone 1g IM
if penicillin allergy:
- oral cefixme + oral azithromycin
screening and gonorrhoea?
screen for other STIs: STI+ HIV screen
Contact tracing
Avoid sex for 7 days until them and partner completes treatment
Follow up patient after 1 weeks for test of cure
