In the past, structural changes to the vasculature were considered the dominant mechanism underlying HPH in healthy lungs
where sustained exposure to sea-level PO2 restored PAP to baseline in chronically hypoxic animal models due to regression of prior remodellinh
However, these observations were primarily based on histological data
with fixed speciments not fully representative of structural features in living organisms
In human sea level dwellers with stabilised HPH at high alt
acute normalisation of alveolar O2 did not substantially alter resistance instead small changes in PAP were attributed to reductions in cardiac output
FM, adminstration of vasodilators such as prostacyclin were not shown to
substantially reduce PVR in chronically hypoxic lungs (Reviewed by Rowan et al. 2016)
