As previously mentioned, hypoxia itself can cause
severe PH with SMS occuring in a portion of otherwise healthy individuals that travel to high altitude
However, the fact that majority of those that travel to high alt
do not experience these negative effects led to the hypothesis that within the normal sea-level population the sensitivity of the pulmonary circulation to hypoxia may vary between individuals
It is argued therefore that if these individuals were to develop COPD that they may be at a higher risk
of right ventricular failure and increased mortaility
This hypothesis suggests the role of epigenetic variability
being the effect of one’s environment on their genetics and how this may result in altered susceptibility to chronic hypoxic lung diseases such as COPD
Another example of potential genetic variability in sea-level natives is
in cows brought to the rockies (>2500m) during summer where susceptible cattle are shown to develop PH resulting in RH failure, oedema (brisket) and death
High alt pulmonary hypertension (HAPH) occurs in about
20% (10-50%) of the animals (Newman et al. 2011)
