what are the 2 types of peri implant disease?
Peri- Implant Mucositis -Reversible inflammation affecting the peri-implant soft tissues without any bony involvement - reversible
and Peri-Implantitis - non reversible
what patients are most likely to have implants?
pts with missing teeth
pts with oral cancer
pts who have had dental trauma
pts with dentures
what is Osseointegration of an implant?
A soft tissue cuff similar to gingivae develops around the Implant
Successful Implants will depend on appropriate patient selection and planning
what causes peri implantitis?
Poor OH
Status of the tissue surrounding the Implant
Impaired host defence
Implant design
Operator placement
Degree of roughness allowing plaque accumulation around the structure
Excessive mechanical forces
how would you monitor tissue around implants? and how would you detect peri implantitis?
Signs of chronic inflammation -colour, texture of gingival tissues
Bleeding on gentle probing
Probing depths increase
Suppuration
Mobility
Radiographic bone loss- progressive loss of bone height and possible angular defects and exposure of implant structure

REFER
If Peri-implantitis is present, the probe will penetrate through the epithelial barrier and connective tissue to Alveolar bone
An increased probing depth will be seen and likely BOP.

Suppuration occurs if collagen is being destroyed and tissue necrosis
what would the treatment be for periimplantitis?
non surgical mechanical therapy using carbon fibre, or titanium instruments, or plastic inserts ultrasonics to minimise damage to the implant surface and superstructure
what is peri implant mucositis and what causes it?
inadequate plaque control,bleeding on probing or palpitation, or when patient is brushing
Pain and tenderness
Swelling/oedema/erythema
Increase in pocket depths as attachment between soft tissue and implant weakens
what are some of the microbiology (bacteria) involved in peri implantitis?
Gram -ive anaerobes e.g;
P Gingivalis , bacteroides forsythus, P Intermedia, Fusobacterium nucleatum AA, Staphylococus
what are factors that affect implant success?
Access for oral hygiene
Poor oral hygiene
Smoking
History of Perio disease
Poorly controlled Diabetes
Occlusal Forces
