Tardieu Scale

A

Spasticity assessment that measures ROM and type of muscle reaction at varying velocities
- V1: as slow as possible
-V2: speed of limb falling (gravity)
-V3: fast

X= Qualify of muscle reaction (0= no resistance, 1= slight resistance, no clear catch, 2= clear catch followed by release, 3=fatigabal clonus (<10sec) 4= infatqiuable Clonus (>10sec) 5= joint immobile)

START IN TH POSITION OF THE MUSCLE YOU ARE TESTING
Example: testing elbow flexion, start in elbow flexion and move into ext.

Strengthen opposing muscle to reduce spasticity
Stretching

NOTTINGHAM SENSORY ASSESMENT (Tactcile)

A

A standardised scale for Ax of sensory impairment

Scoring:
0: Absent, fails to identify on 3 occasions
1: Impaired: identifies test sensation, not on all three occasions
2: normal
9: unable to test

If score 2 on light touch, give 2’s for all pressure & pinprick.

  • Light Touch: Touch not brush, with cotton ball
  • Pressure: Press just enough to deform the skin
  • Pinprick: neurotip mainting even pressure
  • Temperature: hot and cold test tubes (use the side)
  • Tactile localisation: only test where pt has scored 2 on pressure: talcum powder… point exact same spot.
  • Bilateral Simultaneous Touch: touch corresponding sites on one or both sides of the body.

A

Joint tenderness of the hemiplegic shoulder

Supine, body supported. Hemiplegic shoulder is abducted to 30 degrees, elbow extended to 90 degrees flex. and forearm in neutral position.

From this position, rotated to 90°

YOU ARE LOOKING AT THE PT. FACIAL EXPRESSIONS
0 = no tenderness, 1 = complains of pain voluntarily or when asked, 2 = complains of pain voluntarily, wines 3 = complains of pain voluntarily, wines AND withdraws
