Immunocompromised Fungal Infections Flashcards
Inhibition of RNA/DNA synthesis in nucleus
Flucytosine
Inhibition of Egosterol synthesis in cell membrane
Azoles
Fluconazole, Ketoconazole, Itraconazole, Voriconazole, Posaconazole
Inhibition of B-1,3-glucan synthesis in cell wall
Echinocandins:
Caspofungin, Micafungin, Anidulafungin
Ergosterol disruption in cell membrane
Polyenes:-
Amphotericin B
Abelcet (ABLC)
AmBisome (L-AMB)
Amphotec (ABCD)
Polyenes: Amphoterecin B (Fungizone)
1- Given IV
2- Renal toxic —-> Azotaemia
3- Works on Ergestrol
4- Good for Candida albicans and Aspergilles
Fluconazole
1- Works on Ergestrol synthesis
2- Inhibits CP450 (liver) —> so worry about metabolism of other drugs patient is taking
Voriconazole
1- Visual disturbances
2- Drug of choice for Invasive Aspergilles
Flucytosine
1- Inhibits DNA/RNA synthesis
2- Can never be used alone
3- Nephrotoxic
Echinocandins: Caspofungin
1- Most common drug now for Candidemia
2- Inhibit B-1,3-glucan synthesis (‘disruption of cell wall’)
3- Only IV (not oral)
Aspergillosis
Voriconazole
Candidiasis
Caspofungin / Flucanazole
Cryptoccocosis
Amphotericin B + Flucytosine
Zygomycosis (Mucormycosis)
Amphotericin B
Pneumocystitis
Sulfa drugs
Histoplasmosis
Amphotericin B / Itraconazole
