Cyclosporine MOA
Binds to cyclophilins –> Inhibits calcineurin - calcineurin normally activates IL2 transcription
Cyclosporine SE’s
Nephrotoxicity** - vasoconstriction of BOTH afferent & efferent hyperglycemia, HTN, gingival hyperplasia, hirsutisim
Tacrolimus (FK-506)
binds to FK-binding proteins –> inhibits calcineurin **similar outcomes & SE’s as cyclosporine
Sirolimus (rapamycin)
Inhibits mTOR - blocks Tcell proliferation in response to IL-2
Azathioprine
Antimetabolite precursor to 6-MP - blocks synthesis of nucleic acids - toxic to proliferating lymphocytes
Azathioprine SE’s
Bone marrow suppression *metabolized by Xanthine Oxidase
Muromunab (OKT3)
Binds CD3 & prevents T cell signal transduction
Mycophenolate
Inhibits IMP dehydrogenase –> prevents guanine synthesis ****Increases risk of lymphoma****
Thalidomide
Inhibits TNF alpha **used in leprosy
