Hypoxic Ischaemic Encephalopathy Flashcards
1
Q
What is HIE?
A
It is an interruption of the supply of oxygen (hypoxia) or blood flow(perfusion) to the brain
2
Q
What are the causes of HIE?
A
- Compression or kinking of the umbilical cord
- Compression of the placenta
- Placenta abruptio
- Prolonged labour
- Maternal haemorrhage
3
Q
How is HIE phased?
A
There are two phases
- Primary energy failure
- Secondary energy failure
4
Q
What occurs in the primary brain energy failure phase?
A
It usually occurs <6 hours There is a drop in cerebral perfusion leading to -hypoxia -hypglycaemia -anaemia -metabolic acidosis -ischeamia neuronal cell death
5
Q
What occurs in the secondary brain energy failure phase?
A
This occurs after 6 hours Characterised by: -inflammation -apoptosis -oxidative injury -decreased growth factors -impaired protein synthesis
6
Q
What other systems are effected by HIE except for the brain?
A
- Electrolyte derangement
- Pulmonary hypertension
- Respiratory depression
- Cardiac dysfunctional
- Renal impairment- oliguria
- Hepatic impairment
7
Q
How is HIE classified?
A
-according to the Sanrat grading scale
8
Q
What are the symptoms that the baby will present with if they have HIE?
A
- Decreased tone
- Absent pupillary reflex
- Seizures
- Apnea
- Decreased level of consciousness
- Increased intracranial pressure
