Hypertention Flashcards
furosemide (Lasix)
Loop Diuretic Moderate
furosemide (Lasix) MOA
acts on Loop of Henle and inhib reabsorb of Na+/K+/2Cl- which prevents water reabsorption
furosemide (Lasix) AE
Hypokalemia, hyponatremia, hypocalcemia, ototoxicity, hyperglycemia, increased LDLs
hydrochlorothiazide (HCTZ)
Thiazide Powerful
hydrochlorothiazide (HCTZ) MOA
Acts on early part of convoluted tubes; increased Na+ and K+ excreation and reabsorption of Ca+
hydrochlorothiazide (HCTZ) AE
Hypokalemia (extra), hyponatremia, ototoxicity, hypercalcemia, hyperglycemia, increased LDLs
hydrochlorothiazide (HCTZ) Other
Lowers Systolic BP more than other classes; lower risk of kidney stone; favored in older adults bc of Ca+
Better choice for renal Calculi
Spironolactone
Potassium sparing Diuretic
Spironolactone MOA
ihibits Na+ K+ exchange; limits reabsorb of Na+ and secretion of K+
Spironolactone AE
Hyperkalemia, nausea, lethargy, mental confusion Menstral irregularities and gynecomastia in males
PT specific diuretics
Hypokalemia: flattened T waves, muscle weakness, cramps, polyuria Hyperkalemia: elevated T waves, hyperreflexia, numbness Monitor K levels Syncope may be present may produce hyperglycemia Orthostatic hypotension Dehydration Arrythmias DDI - NSAIDS
Amiodipine
Calcium channel blockers
vasodialator
Calcium channel Blockers MOA
Block Ca2+ in vascular smooth muscle
Calcium channel blocker AE
HA, dizziness, hypotension, bradycardia, reflex tachycardia, tremor, flushing, constipation
Metoprolol
Betablocker B1 specific
propranolol
Betablocker
carvedilol
Betablocker B1 specific
Beta blocker MOA
NE binds to B1 and B2 receptors; reduce sympathetic input
Beta blocker AE
Broncoconsriction (only if B2), bradycardia, reduced exercise capacity, abrupt withdrawl can cause arrhythmia, dizziness, OH, depression, fatigue, Sexual dysf.
Beta blocker PT specific
“depresses HR and CO during Exercise
may contribute to OH
watch for CHF (congestive heart failure)
Can hide hypoglycemia in ppl with diabeties “
Linsopril
ACEi
Enalapril
ACEi
Ramipril
ACEi
Losartan
ARB
Valsartan
ARB
ACEi MOA
angiotensin I converting inhibitor
ACEi AE
dry cough, hypotension, hyperkalemia, monitor labs, acute renal failure, angioedema
ACEi PT
cough, NSAIDs
ARB MOA
Angiotensin II receptor blcoker
ARB AE
hypotension, hyperkalemia, monitor labs, acute renal failure, angioedema
ARB PT
NSAIDs
Clonidine
Central Acting Alpha antagonist
Central Acting Alpha antagonist MOA
targets a2 recptors decreasing NE and sympathetic outflow
Central Acting Alpha antagonist AE
Dizziness, drowsiness, fatigue, headache
Overarching therapeutic guidelines for antihypertensive
"OH >> FALLS Dehydration Caution with heat modalities Depleated electrolytes polypharm Heart not responding right to exercise"
