the temptation to form premature theories on insufficient data
What is Base rate neglect?
neglect to keep into account the incidence of the given condition

Settling on the easiest explanation
What is Diagnostic momentum bias?
plan made by a senior remains operational without the clinician on-site acting on the ongoing changes in the patient’s condition.
What are some ways to tackle human factors?
avoid reliance on memory, increasing awareness of biases, routinely use checklists, decrease reliance on vigilance
