What is an AFE?
amniotic fluid entering maternal circulation triggering a serious reaction (cardiorespiratory collapse and/or coagulopathy.)
Why does it happen?
Thought to be due to humoral, haemodynamic and coagulopathic changes - Similar response to anaphylaxis and septic shock.
Other names?
Anaphylactoid syndrome of pregnancy
Sudden obstetric collapse syndrome
Risk factors?
IOL with prostaglandin
maternal age, Csection or instrumental, placenta praevia, Multiple pregnancy
Diagnosis?
acute hypotension, Haemorrhage/Coagulopathy, Acute hypoxia, cardiac arrest/seizure
Coagulopathy?
AFE is almost always associated with a form of disseminated intravascular coagulopathy (DIC).
What is Disseminated Intravascular Coagulation (DIC)?
inappropriate coagulation within the blood vessels leading to failed clotting and haemorrhage
When does it usually happen?
In labour or TOB
What investigations can we do?
low hb, high wbc, X-ray, blood gases
How do we manage it?
ABCD (D- delivery)
What are the outcomes?
fatality 11-30%, 7% brain injury
