i. The insurance or surety company name followed by the policy or bond number
ii. The California Certificate of Deposit number issued to self-insured companies or individuals
iii. Enter “BINDER” whenever evidence of FR is in the form of an insurance document or note stating that an application for insurance has been received and is being processed
iv. Enter “RENTAL” for all rental vehicles regardless of registration
v. Government entities are considered self-insured – write the name of the government entity
vi. If an officer is called away from the enforcement stop due to an emergency before FR information can be obtained, enter “EMERGENCY” on the CHP 215
vii. Enter N/A when:
1. The notice to appear is for stopping, standing, or parking
2. The driver is issued a CHP 215 for a violation other than a CVC violation
3. When the driver is issued a CHP 215 for other than FR and the employer is issued a CHP 215 for Section 16028(a) CVC on an owner’s responsibility CHP 215
