env
gp120 and gp41
Cleaved from gp160
Forms enveloped glycoproteins
gp120/gp41
gp120: Docking glycoprotein (CD4+ tell attachment)
gp41: Transmembrane glycoprotein (Fusion and entry)
Acquired via budding
gag
p24: Capsid protein (Conical capsid)
pol
Reverse transcriptase, aspartate protease, integrase
gp120
binds CD4 and co receptor (CCR5 early, or CXCR4 late)
Homologus CCR5 mutation
Immunity
Heterozygous CCR5 Mutation
Slower course
DX:
Presumptive with ELISA, Sensitive. but not specific.
R/O HIV
Confirm w/ Western Blot (specific)
Look for antibodies to viral proteins. (falsely neg in first 1-2 months) or positive in babies (IgG across placenta)
PCR confirms in window
PCR Viral Load
Determine prognosis, monitor effects of drug, Dx.
AIDS Dx:
Acute Infection:
Wide dissemination, Flu like, Fall in count.
Virus seeding lymphoid organs
Clinical Latency
Feel fine, noticable titer of virus, and stable WBC, albeit lower.
Final Crisis
AIDS
Prophylaxis:
Low grade fever, cough, HSM, tongue ulcer
Fluffly white cottage cheese lesions
Hairy Leukoplakia
Lateral tongue. EBV,doesn’t scrape
Superficial vascular proliferation w/ neutrophils
Bartonella henselae
Doxycycline and macrolides
Chronic, Water Diarrhea
Brain Abscess
Dementia
Many causes: Directly associated w/ HIV
Encephalopathy
Reactivation of JC: PML
Demyelination
Meningitis
India Ink+ Cryptococcus
