Green arrow = hyaline cartilage (type II). Blue arrow = fibrocartilage (type I). Fibrocartilage is more common in bone.

It begins forming at the inner layer of the periosteum where the osteoblasts lie. When the bone is injured or cancerous, these cells begin replicating rapidly and push out the cortical bone.
