What is hepatorenal syndrome?
A
Rapid deterioration in kidney function in individuals with liver failure.

What is the pathophysiology of hepatorenal syndrome?
A
- Vasoactive mediators cause splanchnic vasodilation which reduces sympathetic vascular resistance.
- Causes ‘underfilling’ of the kidneys.
- Activation of RAAS, renal vasoconstriction, not enough to counteract splanchnic vasodilation.

What is the management for hepatorenal syndrome?
A
- Liver transplant ideally
- Vasopressin analogues - Terlipressin
- Volume expansion with 20% albumin
- Transjugular intrahepatic portosystemic shunt (TIPS)
