What are the causes of ascites?
A
- Cirrhosis (75-80%)
- Congestive heart failure
- Neoplasm
- Hypoalbuminaemia

What are the findings on examination in ascites?
A
- Early satiety, SOB (rapid accumulation of fluid)
- Flank bulging and shifting dullness (confirmed by USS/CT/MRI)
- Elevated JVP suggests cardiac cause
- Enlarged Virchow’s node = malignancy
- Pleural effusion? Peritonitis?

How is a suspected ascites investigated?
A
Ascitic fluid tap - albumin, high neutrophils (bacterial peritonitis), gram stain and culture, cytology if cancer suspected.
