Hepatic Portal System Flashcards

1
Q

What is the purpose of the portal venous system?

A

Blood from the spleen, pancreas, and all GI organs are shunted to the liver to be filtered for toxins and nutrients and etc before they can be brought to the systemic circulation.

Q

WHere does the Hepatic portal system start and end?

Q

What is the porta hepatis?

A

A deep fissure on the inferior surface of the liver through which all neurovascular structures (except hepatic veins) and hepatic ducts enter and leave the liver.

16
Q

What drains the genitourinary system?

A

Internal iliac vv.

17
Q

What drains the lower limbs?

A

External iliac vv.

18
Q

In Esophageal Varices, the left gatric vein carries blood to anatamose with what other vein and where does it go from there?

A

Esophageal V. to azygos V and then superior vena cava.

19
Q

What are clinical signs of esophageal varices?

A
  • bleeding
  • black tarry stools
  • medical emergency.
20
Q

In what ligament does the paraumbilical veins run in?

A

falciform ligament.

21
Q

In Caput medusae, what is the caval drainage?

A

Superficial epigastric veins which empty into the external iliac and then to the IVC.

22
Q

Blood back flows from the superior and inferior mesenteric veins to go to the ascending and descending colon. where does the blood go from there to reach the caval system?

A

Retroperitoneal veins that drains the posterior body wall (Veins of retzius (lumbar)

23
Q

In rectal varicies via what vein does the blood back flow and where does it go from the rectum to reach the caval system?

A

Via superior rectal veins to the rectum anastomoses and from there goes through the middle and inferior rectal veins to internal iliac veins to each the caval system.

24
Q

Where is the pectinate line located?

A

2/3 way down into the anal canal.

25
Q

why is the pectinate line a significant landmark in with relation to rectal hemmorroids?

A

Above the pectinate line is visceral innervation and so hemorroids will not be painful. Below the pectinate line is somatic innervation and so hemorroids below the pectinate line will be painful.

26
Q

What is the TIIPS procedure?

A

Transjugular intrahepatic portosystemic shunting. It’s an artifical channel within the liver that establishes communication between the inflow portal vein and the outflow hepatic vein.
