Hemodynamics (S.G) Flashcards
3 hemodynamic lines
- Arterial line (a-line)
- Pulmonary artery catheter (swan ganz)
- Central venous pressure catheter (CVP/RAP)
- Arterial line (A-line)
- what does it monitor?
-arterial blood pressure
Arterial line (A-line) -what can it also be used for?
- non-traumatic sampling of ABGs
- measuring MAP (mean arterial pressure)
Arterial line (A-line) -what CANNOT be infused in this line?
MEDS
how to calculate MAP
3
- Pulmonary Artery catheter (swan ganz)
- what does it measure?
- Pulmonary Artery (PA) pressure
- Pulmonary Artery O. Pressure (PAOP)
Pulmonary Artery catheter (swan ganz)
-is balloon inflated or deflated for PA pressure ?
Deflated
Pulmonary Artery catheter (swan ganz)
-is balloon inflated or deflated for PAOP pressure ?
Inflated (wedge)
*gets LVED reading (left ventricle at end diastole)
Pulmonary Artery catheter (swan ganz)
-what port infuses meds?
Proximal port
Pulmonary Artery catheter (swan ganz)
-what other measurements can be taken?
- C.O readings
- measures core body temperature
- obtains MIXED venous blood sample
Pulmonary Artery catheter (swan ganz)
NORMAL VALUES:
-PA
systolic: 20-30
Diastolic: 5-10
Pulmonary Artery catheter (swan ganz)
NORMAL VALUES:
-PAOP
5-12 mmHg
Pulmonary Artery catheter (swan ganz)
NORMAL VALUES:
-C.O
4-6 L
- CVP catheter (RAP)
- what does it measure?
Pressure in the RIGHT ATRIUM
*shows preload amount of fluid
CVP
-indicates what of the pt?
Fluid balance
CVP
-what can be administered?
FLUIDS and MEDS
CVP
-Normal value
2-5 mmHg
CVP
-cause of a LOW CVP
fluid defecit
- hypovolemia
CVP
-cause of a HIGH CVP
fluid overload
-CHF or decreased C.O
Zeroing an A-line:
Where do you level the transducer?
4th ICS Mid. axillary line (phlebostatic axis)
7 Steps to zeroing an A-line
- level the transducer
- turn stopcock OFF to pt
- OPEN transducer to air
- Press zero on monitor
- CLOSE transducer to air
- turn stopcock ON to pt
- quick flush line 1-2 times
ICP waveforms
-Normal value
4-15 mmHg
ICP waveforms
-slightly elevated
15-20 mmHg
ICP waveforms
-interventions if >20 mmHg for > 10 min
- NOTIFY MD
- Raise HOB
- O2
ICP waveforms
-medications to give if >20 mmHg
- Manitol (diuretic)
- hypertonic saline solution
- consider CO2 and PaO2 levels
ICP waveforms
-Interventions if there is an air embolis
-Reverse trendelenberg (HOB elevated)
IV Drips:
-orders for titrations must include (5)
(MSG-IT)
M: maximum rate S: start rate G: titration goal I: incremements between titration T: time lapse between titration
