Hemodialysis Shepler Flashcards
which pts do we use CAVH, CVVH, or CVVHD for?
AKI or very sick pts
indications for renal replacement therapy (RRT) (5 of them)
AEIOU
Acid base balance
Electrolyte disturbances
Intoxication
fluid Overload
Uremia
general rule to initiate dialysis: BUN > ___ , SCr > ___
100; 10
AV fistula things to know (3 things)
-longest survival rates (approx 20 years)
-fewer complications but takes 1-2 months to mature
-no needlesticks or BP cuffs on access arm
AV graft things to know (3 things)
-involves tubing
-shorter survival than AV fistula
-can use in 2-3 weeks
substances not removed during dialysis (4 of them)
-high Vd
-high lipophilicity
-large MW
-highly protein bound
what does the Kt/V measure?
measures the fraction of total body water that is cleared of urea
what does the urea reduction ratio (URR) measure?
measures reduction of BUN
goal Kt/V for effectiveness of dialysis
1.4 or greater (no units)
goal URR for effectiveness of dialysis
greater than or equal to 70%
complications of hemodialysis (3 of them)
-hypotension
-pruritus
-muscle cramps
