Q

blood supply to the hand is by what two arteries?
what are the arches formed?

A

radial and ulnar arteries
- superficial palmar arch = ulnar artery
- radial artery = deep palmar arch

follow the arteries (deep and superficial venous palmar arches)

14
Q

what pattern do the superificial veins drain into

A

drain into dorsal venous network on back of hand
- cephalic vein = originates from the lateral side of the dorsal venous network
- basilic vein = originates from medial side of dorsal venous netwrok

15
Q

ulnar nerve

A

enters hand lateral to pisiform and posteromedially to ulnar
- deep branch (motor) = innervats ALL intrinsic muscles except thenar muscles
- superificial branch (sensory) = innervates dorsal and palmar of 5 & 4 digits

16
Q

median nerve

A

enters the hand by passing through carpal tunnel
- recurrent branch (motor) innervates thenar muscles and first two lumbricalis muscles
- palmar branch (sensory) innervates the skin on palmar surface of 3 and half digits and dorsal aspects of distal phalanges of same digits

17
Q

radial nerve

A

superficial branch is only part that enters hand
- by passing the anatomical snuff box on dorsolateral side of wrist
- provides senosry innervation over dorsolateral aspect of the palm and dorsal apects of the palm and lateral three digits

18
Q

3 carpal bones at the wrist joint
what are they?

A
  • scaphoid
  • lunate
  • triquetrum
19
Q

carpal tunnel

A

formed anteriorly at the wrist by a deep arch formed by the
- proximal row of carpal bones
- flexor retinaculum (thick connective ligament bridges space between medial and lateral sides of the base of the arch and converts carpal arch into carpal tunnel)

20
Q

carpal tunnel has 9 tendons and one nerve

A
  • median nerve
  • 4 felxor digitorum superficialis
  • 4 flexor digitorum profundus
  • flexor pollicis longus

carpal tunnel has no blood vessels

21
Q

fracture of the scaphoid

A

common fracutre is across the waist of the scaphoid bone
- scaphoid bone has a sole blood supply (radial artery) proximal portion undergoes avascular necrosis

22
Q

De Quervains syndrome

A

inflammatory disorder occurs within first dorsal extensor compartment and involves the extensor pollicis brevis and abductor pollicis longus
- wrist pain preventing flexion/extension and abduction of thumb

23
Q

carpal tunnel syndrome

A

pressure of the median nerve w/in tunnel
- pins and needles sensation
- weakness and loss of muscle bulk of the thenar muscles
- Tinels sign = gently tapping over median to see if pins and needles sensation occurs

24
Q

ulnar nerve injury

A

commonly injuried in elbow and wrist
- elbow = nerve lies posterior to medial epicondyle
- wrist = ulnar nerve passes superifical to flexor retinaculum and lies lateral to pisiform bone
- clawing of the hand
- impaired sensory innervation of palmar aspect of the medial one and one half digits

25
Q

radial nerve injury

A

dropped hand due to deep branch of the radial nerve paralyze muscles of posterior comp
- superificial branch of the radial nerve is little damage only supply small area of skin
