Anaemia of chronic disease
Caused by chronic inflammation, caused by inflammatory cytokines
How does anaemia of chronic disease work?
Available iron is not released for use in the bone marrow, marrow shows a lack of response to erythropoietin, reduced lifespan of RBC
Examples of anaemia of chronic disease
- Rheumatoid arthritis, inflammatory bowel disease, chronic infections
Hepcidin
Protein produced in liver, regulates iron metabolism
How is hepcidin regulated?
cytokines, HFE
Mechanism of hepcidin
Inhibits ferroportin so:
- decreases iron release from macrophages
- prevents iron absorption from gut
Anaemia of chronic disease treatment
- treat underlying condition
- ensure B12, folate and iron stores are adequate
Rheumatoid arthritis treatment
- for pain relief NSAIDs
- Disease Modifying Agents (DMARDs): corticosteroids, chemotherapy
Rheumatoid arthritis and anaemia
GI blood loss due to NSAIDs and steroids
Felty’s syndrome
rheumatoid arthritis, splenomegaly, neutropenia
