habits in kids Flashcards
non nutritive sucking habits
provides comfort and sooth
reduces incidence of SuddenInfantDeathSyndrome before 1 (maybe cos airway is open)
thumb sucking on dentition
post one year old:
open bite
increased overjet (procline upper, retrocline lower)
posterior cross bite
high palatal vault
impact dependent on frequency and duration
other effects from thumb sucking
orthopedic changes, callous, difference in length, herpetic whitlow, middle ear infection
children prefer other children who are not sucking
management of thumb sucking
follow pathway of care
***patient wants to stop
management
counselling and reward system- calendar awards, for mature kids
reminders: band-aid, nail polish, sock, unpleasant taste
adjuct therapy: ace bandage 60% (arm straightens), thumb guard, palatal crib 90% (only works if patient wants to stop), bluegrass appliance 90% effective (works well, gives patient’s tongue something to do), removable appliance (doesnt work cos they can take out)
what are the effects of bruxism
esthetic,
pulp necrosis (rare cos tertiary dentine is laying down, unless patients have neurological defects the bruxes at higher rate)
functional issue
TMD
define bruxing, prev
non functional grinding of teeth, 15% of kids, severe in
causes of childhood bruxing
allergies, occlusal interference, stress (counseling)
how to treat
counseling,
mouth guard: challenges faced in developing dentition, dependent on compliance
refer for botox: injected to masseters, reduce salivary gland drooling lasts for 11 weeks
clonazepam (anti-seizure, not really prescribed)
tongue thrusting causes or effect
as teeth erupt, the incisors allow lip seal and tongue position is backwards
in infantile swallowing, the tongue positioned forward during sucking and tip protrudes through to achieve anterior lip seal
managements
questionable: myofunctional appliances, tongue exercises
orthodontic appliances
