Factors resulting in pyelonephritis/UTI
bubble bath/swimming frequency
recent toilet training (doesn’t wipe properly or not at all)
good hygiene
age = anatomy (short urethra allows bacteria to get in to urinary tract)
urinary stasis
what is urinary stasis?
holding urine to long - not wanting to
not fully urinating, urine remains in bladder (moisture area for bacteria)
s/s of pyelonephritis/UTI in infants
persistent diaper rash - not being changed enough
when a child presents with abdominal pain what should we rule out
abdominal issues
priority order for suspected pyelonephritis/UTI
urinalysis - urine culture (confirm suspicion)
Bacteria seen with pyelonephritis/UTI (80%)
Ecoli
medical mgmt. for pyelonephritis/UTI
urine culture repeated
every month for 3 months
every 3 months for 6 months
then annually
treatment for pyelonephritis/UTI (first 48 hours)
iv antibiotics for the first 48 hours followed by
po antibiotics
why do we want to repeat urine cultures after treatment for pyelonephritis/UTI
preventing recurrence
preventing damage that might occur to the kidneys
is patient at an increased risk for recurrent infections after Pyelonephritis/UTI?
yes - they may be asymptomatic with repeated infections
treatment for subsequent pyelonephritis/UTI infections?
prophylactic antibiotics - don’t want kidney to get damaged
teaching priority to discuss with parent of child with UTI/pyelonephritis?
take all medication as prescribed
