Cardiac Case Study Flashcards
do we change the first dressing after a cardiac cath?
no, cardiologist will do it
what s/s would you EXPECT after a cardiac cath procedure
small amount of dry blood on the dressing
weaker pulses below the cath site
after a cardiac cath what would we want to do if persistent/wet bleeding occurred
*pressure 1 inch above the insertion site of catheter
measure
due to irritation of catheter in ventricles what may be a COMPLICATION of procedure?
may cause a SUSTAINED dysrhythmia/brady
what do we do prior to a cath procedure so we have a baseline
mark pedal pulses pre procedure - nurse knows where to look for pulse
s/s of heart failure
sweating weak thread pulse dependent edema fatigue tachycardia
treatment for heart failure
digoxin (helps heart work better) ace inhibitor (vessels dilate) diuresis (removes fluid) - limit fluids low sodium diet supplemental oxygen keep HOB elevated - better chest/lung expansion
think of heart failure as an
ineffective pump
priority in heart failure
improving cardiac function
if a patient is digitalized/undergoing digitalization what is done?
loading dose of digoxin to bring therapeutic levels up in bloodstream - within 24 hours
lower dose to maintain therapeutic level
**potassium and digoxin relationship
Inverse relationship
If K is high, dig is ineffective
If K is low you become dig toxic
keep within therapeutic levels
is Lasix a potassium sparing diuretic?
yes,
supplement potassium during use
how often should you admin digoxin in peds
every 12hours
what safety measures should we incorporate while on digoxin
take apical HR
monitor therapeutic blood levels
2 RN check
*when should we call the dr w/pt. on digoxin
INFANTS - if HR is less than 90-110
OLDER CHILDREN HR less than 70
